Transference-focused psychotherapy

Transference-focused psychotherapy (TFP) is a form of psychoanalytic psychotherapy that explores deeply buried thoughts and feelings, especially those from childhood. TFP was designed by Otto Kernberg and colleagues in the 1970s and 1980s specifically for people with BPD. As its name suggests, TFP pays close attention to the ‘transference’ between the client and therapist. In therapy, transference refers to how the client feels and thinks about the therapist based on their previous experience of relationships, especially early relationships. For example, someone who was criticized throughout childhood by family members and teachers might experience their therapist as a punishing figure, even when the therapist is being neutral or supportive.

TFP uses the transference to help an individual to better understand themselves and others in order to create lasting personal change. There is evidence to show that TFP can reduce suicidality, self-harm, and help people develop a stronger relationship with themselves and others. It is a highly structured, manualized therapy usually involving one-to-one sessions twice weekly for at least one year.
